ABSTRACT

@#The aim of the present study was to investigate the effects of exosomes derived from bone marrow mesenchymal stem cells (BMSCs) on the polarization of M1/M2 microglia/macrophages in rats with acute cerebral ischemia.Ultrahigh-speed centrifugation was employed to isolate and identify exosomes; a middle cerebral artery occlusion (MCAO) model was prepared in rats using the intraluminal filament technique; Longa scoring and corner tests were used to evaluate the neurological function of rats; 2, 3, 5-triphenyltetrazole chloride (TTC) staining was used to assess the infarct volume in rat brains; immunofluorescence double-labeling of CD16/32/Iba1 and CD206/Iba1 was performed to detect M1/M2 phenotypes of microglia/macrophages; RT-qPCR was employed to measure the mRNA expression of CD86, inducible nitric oxide synthase (iNOS), tumor necrosis factor-alpha (TNF-α), arginase-1 (Arg-1), interleukin-10 (IL-10), and transforming growth factor beta (TGF-β) in the ischemic penumbra of rat brains.The experimental results showed that BMSC-Exos reduced the number of CD16/32+/Iba1+ positive cells in the ischemic penumbra (P < 0.01) while increasing the number of CD206+/Iba1+positive cells (P < 0.01), and decreased the mRNA expression of iNOS, CD86, and TNF-α, while increasing the mRNA expression of Arg-1, TGF-β, and IL-10 (P < 0.05 or P < 0.01).This research suggests that BMSC-Exos can regulate M1/M2 polarization of microglia/macrophages in rats with acute cerebral ischemia, alleviate neuroinflammation, and improve ischemic brain injury.

ABSTRACT

Objective To evaluate the effectiveness of transcranial direct current stimulation (tDCS) in the treatment of depression.Methods The randomized and controlled trials (RCTs) of tDCS in treating depression were searched in electronic databases including Pubmed,Embase,Cochrane library,Wanfang,VIP,CNKI databases all the data until January 2018.All of the studies were evaluated by 2 researchers according to inclusion criteria and exclusion criteria and the data were extracted and assessed by using the RevManS.3 software.Results Totally 11 RCTs studies (n =565 patients) were included in the meta-analysis.The results demonstrated that tDCS was beneficial on depression,with the depression score:[SMD=-0.67,95%CI (-0.84,-0.49),P<0.05],response rate;[OR=2.87,95%CI(1.96,4.20),P<0.05] and remission rate:[OR=1.85,95%CI (1.16,2.96),P<0.05].Conclusion tDCS is of significant effect in treating depression.However,the RCTs included are of a relatively small number of subjects and limited quality,further studies should be focused on the randomized controlled trials of larger sample and higher quality.

ABSTRACT

Objective To investigate the value of rosuvastatin in alleviating early inflammatory reaction and in preventing early recurrence of persistent atrial fibrillation (AF) after catheter ablation treatment. Methods A total of 84 patients with persistent AF who had received catheter ablation were enrolled in this study. The patients were randomly and equally divided into group A (n = 42, study group) and group B (n = 42, control group). Rosuvastatin therapy (10 mg/d, 42 patients) was employed in the patients of group A, while no medication was used in the patients of group B. One month after the ablation treatment, the serum lipid level, hs-CRP and IL-6, as well as the recurrence rate of all kinds of arrhythmia were determined. The preoperative and postoperative levels of inflammatory markers were also determined, the results were compared between the two groups, and its correlation with the early recurrence of AF was evaluated. Results One month after the ablation treatment, the levels of inflammatory markers, including hs-CRP and IL-6, in the study group were significantly lower than those in the control group, as well as lower than those determined before treatment, while in the control group the serum hs-CRP and IL-6 levels became significantly higher than the preoperative ones. Sinus rhythm was successfully maintained in 29 cases (69%) of the study group and in 25 cases (59.5%) of the control group, while the difference was not statistically significant (P = 0.362). The patients of the study group were further divided into non-recurrence sub-group (n = 29) and recurrence sub-group (n = 13). The reduction in inflammatory marker level in the non-recurrence sub-group was greater than that in the recurrence sub-group although the difference was not statistically significant. Conclusion There is remarkable inflammatory reactions during the early period after catheter ablation. Rosuvastatin can effectively relieve the degree of inflammatory reaction, although it has only a slight effect in reducing the early recurrence rate after catheter ablation of AF. The greater the inflammatory markers levels are reduced, the lower the recurrence rate of AF will be, although the difference is not statistically significant.

ABSTRACT

Objective To investigate the effective methods for the treatment of periprosthetic fracture after hip replacement.Methods The clinical data of 13 cases underwent the artificial total hip replacement (THR) with postoperative periprosthetic femur fractures were analyzed.Results The happening time was 12 days to 2 years after primary THR.According to Vancouver classification,there were 2 cases of type A,two cases of type B1,7 cases of type B3,and 2 cases of type C.The treatment included nonoperative treatment in 2 cases,3 cases received open reduction with memory alloy encircling fixer fixation,one case received wire binding with autologous iliac bone grafting,7 cases received revisions with modular revision implant combined with wire binding and autologous iliac bone grafting.The stem was loosened in one type B1 patient at 3 months after surgery.X-ray examination showed that the fractures were united and no infection occurred after operation in 12 patients.No migration or breaking of the instruments was observed.The average Harris hip score was 81 (70 ~ 86points).Conclusion The effective treatment method of periprosthetic fracture after THR should determined by Vancouver classification,degree of bone defect and economic situation of the patients.
